Before starting an internship the following must be completed:
Note: You are not officially enrolled in the Internship Program until all of the items below are completed.
o Application on file
o Transcript on file
o Job description on file (must be submitted within one week of beginning your internship)
o Waiver form on file
Once enrolled in the program, the following items are required before you can receive a grade for course credit:
o Project report proposal submitted (signed by Agency Supervisor and Graduate Director and submitted within the first 75 hours of internship work)
o Project report submitted
o Required hours completed and log submitted
o Work products submitted
o Internship assessment submitted
o Evaluation from Supervisor received in Academic Coordinator's office
